If you wish to register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us.
Please note we are currently not accepting patients who wish to transfer from local GP surgeries. We are only accepting patients newly moved into our catchment area. We will review this position in the future.
Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
Named GP for all patients
All our patients have an allocated named GP to offer continuity of care. This does not mean that in the event of your named GP not being in the surgery you would not be seen - you would be offered an appropriate appointment with an alternative clinician.
GP2GP record transfer
If you register or de-register from Framfield House your records will be transferred electronically with other surgeries who also have this facility in place.
Patients can join our list as Temporary Residents IF they are residing outside their own GPs area. You will be required to sign a Temporary Resident form and give some details of your usual GP and your intended length of stay. You will be required to make an appointment to see a clinician for treatment. Please remember to bring a copy of your last prescription/repeat medication list to this appointment if possible.
